Magnesium is involved in about 600 enzymatic and metabolic reactions (1-7). In patients at risk a magnesium deficiency has to be taken into account, e.g. cardiovascular disorder (2-5).

Diagnosis can made by laboratory, anamnesis (drugs) or clinical symptoms. A magnesium therapy is then often of benefit. Ionized magnesium measurements are a new tool to diagnose a magnesium deficiency (Phox plus apparatus, Nova, Biomedical, USA). This new method can be easily performed in 1 ml blood with a point–of–care unit. Analytic specification for imprecision CV% is only 3.0.

In several studies no correlation between serum and ionized magnesium concentrations in patients existed (3-5).

In conclusion normal serum magnesium does not exclude a magnesium deficiency. The determination of ionized magnesium might be a better tool in this context, e.g. as an early marker for a magnesium deficiency.
